Questionnaires
Questionnaires are clinical rating scales — like the SDQ — that a therapist assigns to your family. You answer them step by step and submit them; the therapist reviews the results as part of your child’s care.
Complete an assigned questionnaire
Section titled “Complete an assigned questionnaire”-
Open the questionnaire from the notification you received when the therapist sent it.

Read the introduction. It shows who requested the scale, any notes they added, and a description of what it’s for.

Answer each question one step at a time. A question selector shows how far you’ve come — question N of M — and which questions already have an answer, and a progress bar tracks the whole scale. Tap Next to move on.

Review the summary of your answers before you finish.

Submit. The questionnaire’s status moves to submitted.
You don’t have to finish in one sitting — a part-done scale is saved as a draft so you can come back to it.
Review submitted answers
Section titled “Review submitted answers”Once a questionnaire is submitted, you can reopen it as a read-only view.

Open the submitted questionnaire.

Read each question together with the answer that was recorded.

Check the details: who requested it and when, who it was assigned to, and when it was completed.
Statuses
Section titled “Statuses”You may see a questionnaire in one of these states:
- Requested — the therapist has sent it and it’s waiting to be filled in.
- Draft — you’ve started answering but haven’t submitted yet.
- Submitted — completed and sent back to the therapist.
When there’s nothing waiting, your pending list shows There are no pending questionnaires.
